Make a referral to the Community Support Team

  

Referral Criteria


 

  • Aged 18+
  • A resident or homeless person in Leeds
  • Someone with severe and enduring mental ill health
  • Someone who is excluded from other services or has been unable to access other services
  • We cannot support people who are in acute crisis at the time of referral
  • We will tell you about other services if the Community Support Team is unsuitable

  

NB - (June 2010): Due to high demand, we have closed our waiting list and are unable to accept any new referrals at this time.  We hope to re-open shortly.

 

Step 1

Complete a referral form.

Email or post it to the team.

Step 2

We will consider whether the person is suitable for the service and notify the referrer of the decision within two weeks of receipt of referral form.

Step 3            

There is usually a waiting list.  We will contact the referrer and the service user when a place is available.
